potential SDOH issues for individual patients based upon a library of SDOH

datasets. Healthcare staff are notified and can

discuss these potential issues with patients, including completing payer

required Health Related Needs Surveys (HRSN).

on this and through the analysis of other data (e.g. clinical, etc.),

SocialHealth360 then creates AI powered “next best actions” for the

patient. These are actioned via SocialHealth360’s

easy-to-use patient navigation workflow that allows the healthcare worker

to assign and track actions (e.g. referrals to community based organizations),

record patients notes and interactions, etc. The solution proactively prompts the

healthcare worker on an ongoing patient to reach out to patients as

required.
